Community Health Improvement Plan

Our services are guided by the Community Health Improvement Plan (CHIP). The CHIP is developed from the Community Health Assessment (CHA), including a survey and focus groups, that is conducted every six years. With the help of community collaborators, the CHA data informs the goals we will prioritize over the next six years. The CHIP details the interventions we plan to implement toward reaching those goals.
The current CHIP will launch October 3! The plan focuses on three priorities, each of which is monitored by its own steering committee.
Following are the three priorities we will focus on over the next six years:
(1) Mental Wellbeing
a. Improve access to mental health care
b. Decrease prevalence of factors contributing to poor mental health outcomes
(2) Chronic Disease Conditions
a. Improve understanding and access to screening and prevention resources
b. Improve access to and navigation of the health system (options, services, costs, insurance)
(3) Senior Health
a. Improve access to resources to maintain successful independent living
b. Increase socialization and physical/social support
