A community health worker is a frontline public health worker who is a trusted member of and/or has an unusual close understanding of the community served. 

South Heartland District Health Department has two community health workers that connect our community to resources in the area to help make happy healthy individuals.  Both community health workers are lifestyle coaches for our evidence-based diabetes prevention program, Smart Moves.

Liz Chamberlain provides health coaching and navigation for chronic disease management, serves as a Healthy Heart Ambassador for the YMCA’s Self-Monitoring Blood Pressure Program.   liz.chamberlain@shdhd.ne.gov

Julia Sarmiento serves as a bilingual community health worker, engaging and empowering individuals in accessing healthcare and making decisions about their health.  Julia also serves as a Healthy Heart Ambassador for the YMCA’s Self-Monitoring Blood Pressure Program.  Julia.sarmiento@shdhd.ne.gov
